New Patient Information

WELCOME TO THE CLINIC!

Here are a few things you need to know before your first appointment. Please call or email Heather if you have any further questions.

Location: The clinic is located at 2216 SE 50th Ave. Portland OR, 97215. There is a large red sign out front that says "River East Building". On street parking is available. Please see our 'directions' page for more information.

Hours: Heather Ellsworth L.Ac. is in private practice mondays, wednesdays and fridays. The schedule is generally 9-5, however some early morning or evening appointments are available in cases of special need. Contact Heather for more information.

Before your visit:
  • Please have your new patient paperwork filled out before your appointment so we can spend more time on your treatment. You can download it on this website, or arrive 15 min. early to fill it out at the office.
  • Wear loose fitting, comfortable clothing to your session. It is often not necessary to completely undress during an acupuncture session but the clothing must roll up past the elbows and knees. Draping is available as needed.
  • For shiatsu massage please wear cotton or stretchy clothing that you have full range of movement in. Yoga or sweat pants and a long sleeved shirt are a good idea.
  • It is best to have a light meal or snack an hour or two before a session to avoid becoming dizzy or nauseous. Try to avoid eating a very large meal right before a treatment.
Acupuncture induces a state of peaceful relaxation, it is best to not have to rush anywhere right after a treatment.

Payment: We accept cash or check at time of service, we are also happy to bill your insurance in which case co-pay at time of service will be due.

Insurance: We are happy to bill your insurance. When you call to make an appointment we can collect your insurance information and call the insurance company in advance of your appointment to check your coverage. The information you will need is on your insurance card: name, date of birth, ID#, and the practitioner phone number from the back of your insurance card.

Cancellations: Please note that your appointment time is reserved just for you. Any cancellation or change in your appointment requires 24 hours notice. Because things happen the first missed appointment is 'forgiven'. After that, all missed appointments will incur the full session fee.
